Witnesses are being sought following a two-vehicle collision near Ruthin.
North Wales Police launched the appeal on Sunday morning, with the incident having taken place the previous day.
A police spokesman said: "We're appealing for witnesses following a collision between two vehicles yesterday on the A494 near Llanbedr DC at 1.05pm - involving a silver Citroen C4 and a red Mercedes A class.
"Two people with serious injuries were transferred to hospital."
